The following disclosures are made by the Prostate Cancer Institute in conjunction with any patient referrals made to physicians and/or the associated facilitation of appointments for patients with those physicians: 1. Physicians are licensed and board certified prostate cancer specialists with hospital admitting privileges who are qualified for participation in the Prostate Cancer Institute referral program. Qualification is evaluated through an assessment of the following: experience in the treatment of prostate cancer, surgical experience for prostate cancer and evidence of training or experience necessary to perform cryotherapy or brachytherapy procedures. 2. The Prostate Cancer Institute imposes no requirements on the manner in which a physician provides services to a patient. Patients are referred to specific physicians based solely upon geographic proximity of the physician to the patient. 3. The Prostate Cancer Institute is not owned or controlled by the physicians to whom referrals are made. The Prostate Cancer Institute may establish a contractual relationship with participating physicians through which appointments for prostate cancer patients may be scheduled. 4. Physicians may discontinue participation by written request to the Prostate Cancer Institute, or may be removed by the Prostate Cancer Institute, if it learns of a material change in any of a physician's participation or qualification requirements.
